This spreadsheet should be used for developing an action plan with a student to state grade goals, demonstrate how those goals will improve GPA, and other supportive activities for student to complete during the quarter to ensure academic success

Academic Action Plan Procedures
Students receive this document in their Dean's office email/letter,
informing them of the steps they must complete before meeting with you as the advisor. Then, you will develop a contract with them as a result. This helps the student
reflect on their situation, and explain it to you completely.
NOTE: what the student brings to the advisor is NOT the
action plan/contract.
First-time Freshman Contract Process
A two-sided document, which the first side is completed by the student before
meeting with the advisor. The second side is a contract template
that should be completed during the advising appointment. This is
used with our first-year students on AP after their first quarter, and serves as an early intervention program. It is compulsory and they will have a hold on their registration for the spring quarter if they do not comply.

students in the process of good academic advising. A two-sided
document that can be printed and kept in a convenient location for
frequent use.
